SOCIAL WORKER PRN

Job Description

We have an exciting opportunity for a PRN Social Worker to join the team at LifeBridge Health. As a crucial member of our organization, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a variety of challenges that may arise in a healthcare setting. This role involves assisting individuals with issues such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.

Responsibilities

• Provide clinical social work services to patients and families.
• Conduct psychosocial assessments and address common challenges patients may face.
• Offer support and counseling related to various issues, including aging and end-of-life preparation.
• Assist with discharge planning and implementation.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed Social Worker (LBSW/LMSW/LGPC).
• Knowledge of clinical social work theory and practice.
