Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Assist with discharge planning and end-of-life care.
• Address financial concerns, family relationships, and emotional challenges.
• Work collaboratively with healthcare teams to ensure holistic patient care.

Requirements

• Bachelor's degree in Social Work.
• 1-3 years of relevant experience.
• Licensed as LBSW/LMSW/LGPC.
• Compassionate and empathetic with strong communication skills.
