Job Description

We are seeking a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide essential clinical services and psychosocial assessments to patients and their families, addressing a range of common challenges such as adjustment to institutional living, financial concerns, and counseling on end-of-life issues.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Assist patients and families with a variety of challenges.
• Engage in discharge planning and counseling related to death and dying.

Requirements

• Bachelor's degree and professional knowledge in social work.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
