SOCIAL WORKER PRN

Job Description

We are looking for a passionate and dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients, residents, and families, addressing various challenges they may face.

Responsibilities

• Provide psychosocial assessments to identify patient/resident needs.
• Offer counseling and support for adjustment difficulties, financial concerns, and end-of-life issues.
• Collaborate with healthcare teams to develop discharge plans.
• Assist individuals in coping with medical procedures and diagnoses.

Requirements

• Bachelor's degree and 1-3 years of experience in social work.
• Licensed as LBSW, LMSW, or LGPC.
• Strong understanding of social work theory and practice.
