Job Description

We are looking for a dedicated SOCIAL WORKER PRN to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various common concerns. The position involves assisting individuals with adjustment to institutional living, financial issues, family dynamics, medical understanding, aging-related changes, discharge planning, and end-of-life counseling.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Address adjustment issues, financial concerns, and family relationships.
• Assist in medical understanding and acceptance of procedures.
• Offer support for aging-related changes and end-of-life counseling.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
