SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients, residents, and families, addressing a variety of common challenges such as adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.

Responsibilities

• Provide clinical social work services and psychosocial assessments to patients and families.
• Address common challenges related to adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.
• Assist with discharge planning and provide support to patients and families.

Requirements

• Bachelor's degree in Social Work.
• 1-3 years of experience in social work.
• Licensed as LBSW, LMSW, or LGPC.
• Knowledge of theory and practice within a specialized field.
