SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health located in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, focusing on various common challenges such as adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying, among others.

Responsibilities

• Offering psychosocial assessments to patients and families.
• Addressing a range of issues including adjustment to medical procedures, discharge planning, and counseling.
• Collaborating with healthcare teams to provide comprehensive care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC certification.
