SOCIAL WORKER PRN

Job Description

We are currently looking for a PRN Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life planning.

Responsibilities

• Offer psychosocial support to patients and their families.
• Assess and address common social and emotional issues faced by patients.
• Provide counseling for patients and families dealing with end-of-life issues.
• Collaborate with healthcare teams to ensure comprehensive care for patients.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC certification.
• Strong communication and interpersonal skills.
