Job Description

We are looking for a dedicated and compassionate PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. The Social Worker PRN will provide clinical social work services and psychosocial assessments to patients, families, and residents dealing with various challenges related to institutional living, aging, discharge planning, and more. Responsibilities

• Provide clinical social work services to individuals and families.
• Conduct psychosocial assessments to address common issues related to healthcare, aging, and family dynamics.
• Offer counseling and support for end-of-life care and bereavement.

Requirements

• Bachelor's degree in social work.
• 1-3 years of experience in a similar role.
• LBSW, LMSW, or LGPC certification.
