Job Description
We are looking for a compassionate and dedicated Social Worker to join our team at LifeBridge Health, specifically for the Baltimore County/City area. In this full-time position, you will be responsible for delivering professional and comprehensive services to individuals in need, ensuring their psychosocial, environmental, and financial needs are met. As a Social Worker, you will collaborate with patients, families, and interdisciplinary team members to develop and evaluate personalized care plans, while also serving as an educator and expert practitioner in the field.
Responsibilities
- Assess patients' and families' social, emotional, and financial situations.
- Develop and implement personalized care plans in coordination with the interdisciplinary team.
- Provide support, education, and counseling to patients and families.
- Maintain accurate and detailed documentation of patient interactions and progress.
Requirements
- Master's Degree in Social Work from an accredited institution.
- Certification as a Licensed Certified Social Worker (LCSW).
- Strong interpersonal communication skills.
- Ability to work collaboratively in a fast-paced healthcare environment.
