Job Description
We are looking for a dedicated Licensed Practical Nurse (LPN) to fill full-time, part-time, or per diem positions in Home Health Care. As part of our team, you will provide skilled nursing care and treatment to geriatric patients in the community, ensuring patient-centered care aligned with available resources.
Responsibilities
- Deliver skilled nursing care and prescribed treatments to geriatric patients.
- Execute care plans that are goal-oriented and patient-centered.
- Participate in discharge planning for patients.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
