Job Description
We are looking for a passionate PRN Social Worker to join our dedicated team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a range of common issues such as adjustment difficulties, financial concerns, discharge planning, and counseling related to aging and end-of-life care.
Responsibilities
- Provide clinical social work services to patients and families.
- Conduct psychosocial assessments and address a variety of social and emotional issues.
- Collaborate with healthcare professionals to ensure comprehensive care for patients.
- Offer supportive counseling and guidance to individuals and families facing challenging situations.
Requirements
- Bachelor's degree with LBSW/LMSW/LGPC certification.
- 1-3 years of relevant experience in social work.
- Strong knowledge of social work theory and practice.
