SOCIAL WORKER PRN

Job Description

We are looking for a skilled Social Worker on a PRN basis to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families. Your responsibilities will include addressing various issues such as adjustment to institutional living, financial concerns, family relationships, and counseling related to medical procedures and end-of-life care.

Responsibilities

• Conduct psychosocial assessments for patients/residents and their families.
• Provide counseling and support for patients facing difficulties.
• Assist with discharge planning and coordination of care.
• Collaborate with healthcare professionals to ensure holistic care delivery.

Requirements

• Bachelor's degree in social work or related field.
• 1-3 years of experience as a Social Worker.
• LBSW/LMSW/LGPC certification.
• Excellent communication and interpersonal skills.
