SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a valued member of our Allied Health team at Levindale Hospital, you will be offering clinical social work services and psychosocial assessments to patients/residents and families, assisting with a range of common issues such as adjustment to institutional living, financial concerns, family relationships, and discharge planning.

Responsibilities

• Provide clinical social work services and psychosocial assessments to patients and families.
• Address a variety of challenges including adjustment to institutional living, financial concerns, and discharge planning.
• Offer counseling and support related to aging, powerlessness, and end-of-life care.
• Collaborate with interdisciplinary teams to ensure comprehensive care for patients and families.

Requirements

• Bachelor's degree and 1-3 years of experience in social work.
• LBSW/LMSW/LGPC certification.
• Strong knowledge of social work theory and practice.
• Compassionate nature and excellent communication skills.
