Job Description

We are looking for a PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. This role involves providing clinical social work services and psychosocial assessments to patients and families, addressing various issues such as adjustment to institutional living, financial concerns, family relationships, and counseling related to medical procedures and end-of-life care.

Responsibilities

• Deliver clinical social work services and psychosocial assessments.
• Address common patient and family problems related to institutional living, aging, discharge planning, and end-of-life care.
• Work collaboratively with healthcare teams to provide comprehensive care.

Requirements

• Bachelor's degree with 1-3 years of experience.
• Licensed as LBSW, LMSW, or LGPC.
• Strong understanding of social work theory and application.
