Job Description
We are looking for a compassionate and dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and their families. This includes addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and counseling related to difficult life situations.
Responsibilities
- Provide clinical social work services to patients and families.
- Conduct psychosocial assessments to identify common issues and challenges.
- Offer counseling and support related to aging, discharge planning, and end-of-life care.
- Collaborate with healthcare teams to ensure the overall well-being of patients.
Requirements
- Bachelor's in Social Work (BSW) required, Master's degree preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
- Strong communication and interpersonal skills.
