Job Description
We are looking for a compassionate and dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and their families. Your responsibilities will include addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, aging-related changes, and counseling related to end-of-life issues.
Responsibilities
- Provide clinical social work services and psychosocial assessments.
- Assist patients and families with various social and emotional issues.
- Conduct discharge planning and provide counseling related to death and dying.
Requirements
- Bachelor's in Social Work (BSW) required, Master's degree preferred.
- LCSW-C preferred.
- LGSW certification.
