Job Description
We are seeking a dedicated Social Worker to join our compassionate team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a wide range of common issues including adjustment to institutional living, financial concerns, family relationships, and counseling related to aging and end-of-life care.
Responsibilities
- Provide clinical social work services and psychosocial assessments to patients and families.
- Address various challenges faced by patients, including adjusting to institutional living and financial concerns.
- Offer counseling related to aging, end-of-life care, and other emotional needs.
- Contribute to discharge planning and assist patients in navigating medical processes.
Requirements
- Bachelor's in Social Work (BSW) required, Master's degree preferred.
- LCSW-C preferred, LGSW certification required.
- Experience in a healthcare setting is a plus.
