Job Description
LifeBridge Health is seeking a dedicated Social Worker to join our compassionate team at Levindale Hospital in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.
Responsibilities
- Provide psychosocial assessments to patients and families.
- Offer counseling and support for adjustment and coping strategies.
- Participate in discharge planning and end-of-life care discussions.
Requirements
- Bachelor's in Social Work (BSW), Master's degree preferred.
- LCSW-C certification preferred.
- LGSW certification.
