Responsibilities
- Conduct psychosocial assessments for patients and their families.
- Address issues such as adjustment to institutional living, family relationships, and end-of-life care planning.
- Provide counseling and support for patients facing medical diagnoses and treatments.
- Collaborate with healthcare teams and assist in discharge planning.
Requirements
- Bachelor's degree in Social Work (BSW); Master's degree preferred.
- LCSW-C preferred.
- LGSW certification.
- Strong communication and interpersonal skills.
