Job Description
We are looking for a compassionate and dedicated Social Worker to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients, residents, and their families, addressing various challenges they may face. This includes assisting with adjustment to institutional living, financial concerns, family relationships, coping with medical conditions, discharge planning, and end-of-life counseling.
Responsibilities
- Provide clinical social work services to individuals and families.
- Conduct psychosocial assessments and develop appropriate care plans.
- Assist patients and families in coping with various issues and challenges.
- Collaborate with healthcare professionals to ensure comprehensive care.
Requirements
- Bachelor's in Social Work (BSW); Master's degree preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW).
