Job Description
We are looking to hire a dedicated Social Worker to join our compassionate team at LifeBridge Health. As a valued member of the hospital, you will provide essential clinical social work services and psychosocial assessments to patients and their families, addressing various challenges they may encounter throughout their healthcare journey.
Responsibilities
- Offer psychosocial support to patients and families dealing with adjustment issues, financial concerns, and family dynamics.
- Provide counseling and guidance on medical conditions, treatments, aging, powerlessness, and end-of-life care.
- Participate in discharge planning to ensure smooth transitions for patients.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- Preferred certification as LCSW-C or LGSW.
- Strong interpersonal and communication skills.
- Empathy, understanding, and commitment to supporting patients and families.
