Your responsibilities will involve using an escalation protocol to identify barriers hindering timely discharges, collaborating with the interdisciplinary team to ensure adherence to discharge planning standards, and proactively seeking early intervention opportunities for cost-effective discharges.
Requirements
- Master's degree equivalent
- Licensed Master Social Worker (LMSW)
