Team Select Home Care is seeking Licensed Practical Nurses to join our family, providing quality home health care to our valued patients. As part of our collaborative team, you will document and deliver skilled nursing care, implementing the plan of care and physician orders while maintaining confidentiality and professional boundaries. You will also prepare clinical and progress notes, assist the physician and RN in specialized procedures, and more.

Responsibilities

• Furnish services, treatments, and diagnostic procedures as assigned per the care plan.
• Prepare equipment and materials for treatments observing aseptic technique.
• Assist patient with learning appropriate self-care techniques and involve in implementing the plan of care.
• Implement the nursing care plan for each patient and initiate preventive and rehabilitative procedures.
• Observe signs and symptoms, report reactions to treatments and changes in patient's condition to the physician and RN.
• Educate and counsel the patient and family/significant others regarding care needs and related problems.
• Evaluate the effectiveness of nursing service under the guidance of the RN.
• Maintain complete records of observations, treatments, and care of the patient, updating the care plan.
• Participate in patient care conferences and perform other tasks as assigned.

Qualifications

• Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing.
• Maintains current CPR certification and BLS (Basic Life Support) Certification (preferred).
• Pediatric experience (preferred).
