Job Description

With more than 35 locations across eleven states nationwide, Team Select Home Care aims to provide exceptional in-home patient care with new methods of reducing re-hospitalizations and driving clinical outcomes.
Responsibilities
Life as a Team Select Hero: Start your day by touching base with your care team. Throughout the day, you will utilize your skills independently to provide 1:1 care to your patient in the comfort of their home, ensuring they are well taken care of.
- 1:1 patient care
- Working with pediatric/adult patients with g-tube, trach, and/or ventilator
- Documenting electronically
- Patient assessments
- In-person
- Developing meaningful long-term relationships

Qualifications, Licenses, and Certifications
- Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing
- Maintains a current CPR certification
- BLS (Basic Life Support) Certification (preferred)
- Pediatric experience (preferred)
