INPATIENT CASE MANAGER (RN)

Job Description

We are looking for a dedicated Inpatient Case Manager (RN) to join LifeBridge Health at Northwest Hospital in Randallstown, MD. As an Inpatient Care Manager, you will work collaboratively with the clinical team and medical providers to deliver discharge planning coordination and intervention for patients with complex health needs. Your role will focus on promoting patient wellness, enhancing care outcomes, optimizing health service utilization, and reducing payment denials.

Responsibilities

• Minimize denials of payment within a patient population with complex health needs.

Requirements

• Basic professional knowledge with a Bachelor's degree or equivalent.
• BSN required from CCNE accredited schools.
• Experience in a specialized field is a plus.
